NCHC's Medical Mobile Unit (MMU) will be available to provide 15 min appointments for physicals.  Please complete the form below and an NCHC representative will make contact for more information and to confirm the appointment requested.

Every Thursday, starting June 9th, the Norwalk Community Health Center’s Medical Mobile Unit will be available at the NPS Family Center for student physical exams and vaccinations. The Mobile Unit will provide a convenient way for families to get student physicals and vaccinations that are required for children to start school.

In Connecticut, students are required to have physical examinations in the following grades:

  • Every year of Pre-K
  • Prior to entering Kindergarten
  • Grade 6 (To be completed after Jan. 1st of your child’s Fifth Grade year)
  • Grade 10 (To be completed after Jan. 1st of your child’s Ninth Grade year)

Appointments for the Mobile Unit at the NPS Family Center will be available from 5 pm to 7 pm every Thursday through mid-September.  The Family Center is located at 1 Park Street, at the corner of Park and Wall Street.

The State of Connecticut Health Assessment form must be completed by a physician with all required immunization dates noted.  The NCHC mobile unit will have forms available during appointments.

Hours of operation for NCHC are Mon-Fri 8:00-5:00 and Saturday 8:00-12:00.
