A pediatrician will provide professional medical care and treatment services for Norwalk Community Health Center (NCHC) pediatric patients. The Physician shall perform all professional duties in accordance with corporate policies and procedures, as well as established standards of care.

  • Provides patient care visits in an outpatient setting, in an organized and efficient manner
  • Provides comprehensive care for a patient panel, seeing patients for both acute care and scheduled visits
  • Collaborates with Mid-Level Practitioners as needed.
  • Evaluates patient problems thoroughly and appropriately
  • Prescribes or plans appropriate and effective treatment
  • Provides clear documentation of patient visit notes, orders, telephone calls, clinical flow sheets and other records as required by quality standards
  • Demonstrates excellent communication skills. Effectively communicates with patients, families and other staff members.
  • Demonstrates sound judgment and decision-making skills and practices within his/her scope of expertise and licensure.
  • Consults with other physicians appropriately; knows when to seek advice. Refers patients requiring inpatient services to Hospitalist Service, performing appropriate hand-off of care and following up with the patient after discharge from hospital.
  • Demonstrates teamwork by working effectively with all team members, attending and participating actively in all-staff, department, and quality meetings

Qualifications:

Education:

Must have successfully completed medical training at an accredited medical school and received an M.D. or D.O. degree as a Pediatrician.

Licensure/Certification:

  • Must be licensed to practice Medicine in Connecticut.
  • Must be Board-Certified or Board-eligible

Knowledge, skills and abilities required:

  • Extensive knowledge of the techniques and skills necessary to diagnose and properly treat diseases and injuries.
  • A working knowledge of the administrative practices and procedures, rules, regulations and policies that govern the rendering of responsible medical and nursing care.
  • A working knowledge of public health and preventative care guidelines.
  • Excellent communication skills.
  • The ability to relate with warmth and compassion to patients and co-workers.
  • The ability to effectively supervise professional and non-professional employees in their clinical duties.

Qualifications

NCHC is a Federally Qualified Health Center (FQHC). Physicians working for an FQHC are eligible for up to $50,000 through the Federal Loan Repayment Program.

We offer flexible schedules with general daytime hours Monday – Friday, with option of up to 3 evenings per week and Saturday hours.

Norwalk Community Health Center, Inc. (“NCHC”) seeks a full-time Business Intelligence Analyst to: review, compile and evaluate complex sets of data and information, ranging from Electronic Medical Health Records and insurance claims to social determinants of health and wearable device data, needed to develop strategies for NCHC, including its operations as Smilow Life Center, to operate more efficiently and effectively and to improve health outcomes; lead the process of collecting, managing and reporting on new organizational data; generate reports used to ensure that NCHC’s funds are most effectively spent on the optimization of operations and healthcare delivery efforts; develop population health strategies that present a common understanding of what various data means so that insight can be gleaned and shared among NCHC’s board members and management personnel; establish Key Performance Indicators (“KPIs”) and create dashboards using established KPIs to measure the effectiveness of business decisions and evaluate the success of the organization and give weekly presentations and summarize monthly organization performance.

 

The minimum requirements for the Business Intelligence Analyst position are: possession of a U.S. Master’s degree in Information Technology, Big Data or Mathematics and proficiency with: programming languages, such as SQL, Python and/or R; data gathering, cleaning, transforming and warehousing techniques; and the ability to use reporting and data visualization software, such as Tableau, as demonstrated by completion of academic coursework in the field of Information Technology, Big Data or Mathematics; and prior work experience in any occupation involving the routine use of such programming languages and reporting and data visualization software.

Summary:

The Operations Coordinator consults with and advises the Chief Executive Officer of challenges relating to the procedures of health center. The position is also responsible for overseeing and organizing the overall operations of the health center front desk, call center, and scheduling staff. The role recommends changes in administrative oversight; provides leadership as a liaison with the operations team; all while delivering discreet and confidential support directly to the CEO.

 

Responsibilities:

• Directs call center and front office operations to maximize patient satisfaction, patient access, verification of benefits, collection of payments and customer service efforts
• Acts as the connection between the clinical and operations teams
• Assists the teams with project timelines to ensure that all milestones and deadlines are met as determined by the CEO
• Ensures and maintains the environment to comply with regulatory, licensure, compliance and accreditation requirements
• Monitors time tracking system for assigned associates approving timecards and requests for time off,
• Works with providers and clinical leadership to ensure accurate scheduling of patient appointments
• In consultation with providers, makes certain that templates in EMR are maintained to provide proper coverage including when schedules change, or accommodations need to be made
• Serves as a management point of contact for patient and external customer complaints
• Responsible for investigating complaints relating to practice operations
• Supports leaders and team members in navigating digital systems and platforms; including but not limited to: the electronic medical record (eCW), Microsoft 365, learning management, performance management software’s and applications; etc.
• Collaborates with all NCHC management in the planning, scheduling, coordination and reporting of projects, goals and objectives
• Maintain thorough organization of documents and digital files, ensuring compliance with regulatory standards as necessary
• Other duties as assigned
